| dc.description.abstract | โรงพยาบาลตราดเป็นโรงพยาบาลทั่วไปที่ให้บริการในระดับทุติยภูมิ รับรักษาผู้ป่วยที่มีการใช้ยาต้านการแข็งตัวของเกล็ดเลือด (วาร์ฟาริน) ในจังหวัดตราด โดยเป็นโรงพยาบาลแม่ข่ายที่รับการส่งต่อผู้ป่วย จากโรงพยาบาลชุมชน และส่งผู้ป่วยกลับให้โรงพยาบาลชุมชนเป็นผู้ดูแลรักษาต่อ ด้วยเหตุที่ผู้ป่วยที่มีการใช้ยาวาร์ฟารินต้องมีการปรับขนาดยาติดตามค่าอัตราส่วนที่เรียกว่า International normalized ratio (INR) ให้อยู่ในช่วงที่กำหนดในแต่ละโรคเป็นเรื่องที่สำคัญสำหรับการดูแลผู้ป่วยที่ได้รับยาวาร์ฟาริน การศึกษาค้นคว้าอิสระนี้จึงมีวัตถุประสงค์เพื่อจัดทำคู่มือการปรับขนาดยาต้านการแข็งตัวของเลือดและผลการศึกษาพบว่า คู่มือการใช้ การปรับขนาดยาและการใช้แอปพลิเคชั่น สำหรับเภสัชกรกลุ่มงานเภสัชกรรม โรงพยาบาลตราดแบ่งเป็น 4 บทคือ บทที่ 1.บทนำความสำคัญของการปรับขนาดยายาด้านการแข็งตัวของเกล็ดเลือด (วาร์ฟาริน) ตามค่าอัตราส่วนที่เรียกว่า International normalized ratio (INR) สำหรับการดูแลผู้ป่วยที่ได้รับยาวาร์ฟาริน โดยเภสัชกร กลุ่มงานเภสัชกรรม บทที่ 2. หลักการและมาตรฐานการปฏิบัติงานที่เกี่ยวข้องกับการปรับขนาดยาวาร์ฟาริน สำหรับเภสัชกรสภาเภสัชกรรม บทที่ 3. ขั้นตอนในการจัดทำ การใช้ คู่มือ และการใช้แอปพลิเคชั่น การปรับขนาดยาต้านการแข็งตัวของ เกล็ดเลือด (วาร์ฟาริน) สำหรับเภสัชกร พร้อมคู่มือการใช้งาน และ บทที่ 4.การปรับขนาดยาให้เป็นไปในแนวทางเดียวกันตามมาตรฐานของสภาเภสัชกรรม สำหรับเภสัชกร จากการใช้คู่มือและการใช้แอปพลิเคชั่น ผลของการใช้คู่มือและแอปพลิเคชั่น คือเภสัชกรกลุ่มงานเภสัชกรรม โรงพยาบาลตราดสามารถปรับขนาดยาต้านการแข็งตัวของเกล็ดเลือด(วาร์ฟาริน)ติดตามค่าอัตราส่วนที่เรียกว่า INR ให้อยู่ในช่วงที่กำหนดในแต่ละโรคของผู้ป่วยโดยไม่เกิดอาการไม่พึงประสงค์ในผู้ป่วยจากการได้รับขนาดยาที่ไม่ถูกต้อง | th_TH |

| dc.description.abstractalternative | Trat Hospital is a general secondary health health-care facility that also treats patients with warfarin (anticoagulant) medication. As a referral hospital, it takes referred cases from community hospitals for treatment and sends them back to referring hospitals for continuing treatment. Each patient on warfarin therapy must have his/her international normalized ratio (INR) monitored to ensure that the INR is within the target INR range for the disease. So, this study was conducted, aiming to create two manuals : one for warfarin dosage adjustments and the other being a “Warfarin Guide” application for smartphones, both for pharmacists in the hospital’s Pharmacy Group. The manuals’ preparation involved the review of related research documents, Pharmacy Council’ s standards for preventing and reducing risks of severe complications related to warfarin therapy, and manual drafting. The draft manuals were examined to ensure content validity and tried out with relevant pharmacists; and then they were revised/finalized for convenience in actual use. As a result of this effort, the Manuals on Warfarin Dosage Adjustments and Warfarin Guide Application for Pharmacists at Trat Hospital have been prepared, containing four chapters: (1) Introduction, importance of warfarin dosage adjustments for pharmacists, based on the INR approach; (2) Principles and operational standards related to warfarin dosage adjustment for pharmacists, based on the Pharmacy Council’s guidelines; (3) Steps for creating and using the manual as well as “Warfarin Guide” application for warfarin dosage adjustments for pharmacists, and the user’s manual; and (4) Warfarin dosage adjustment guide for pharmacists as per the Pharmacy Council’s standards. With the use of the manual and the application, pharmacists at Trat Hospital’s Pharmacy Group are able to adjust warfarin dosages, so that the patients’ INRs are within the range that no adverse drug reactions will occur due to incorrect medication dosage. | en_US |
