The Development of a Clinical Nursing Practice Guideline for Care of Hypertension Patients with Atrial Fibrillation in the Community, Loei Province

Abstract

The purposes of this study were 1) to study the problems and needs of hypertensive patients with atrial fibrillation; 2) to develop a clinical nursing practice guideline (CNPG) for the care of hypertensive patients with atrial fibrillation in the sample community; and 3) to study the effectiveness of the developed CNPG for the care of hypertensive patients with atrial fibrillation in the community.The research design was a research-and-development model. A clinical nursing practice guideline (CNPG) was developed according to Soukup’s concept. The sample group was divided into four steps: 1) Evidence-triggered phase: the sample consisted of 12 patients and 12 registered nurses; 2) Evidence-support phase: the sample consisted of 11 textbooks and research papers, 12 registered nurses, and 5 experts; 3) Evidence-observed phase: the sample group consisted of 15 patients and 15 registered nurses; and 4) Evidence-based phase. The samples for every phase were purposefully selected. Research tools included: 1) a focus group guideline for exploring patient problems and needs; 2) a brainstorming guideline for developing clinical nursing practice guidelines; 3) The Appraisal of Guidelines Research and Evolution II: AGREE II; 4) questionnaires of knowledge and self-care behavior of patients; and 5) a satisfaction questionnaire for the registered nurses about use of the CNPG. Data were analyzed by descriptive statistics, content analysis, and the Wilcoxon signed rank test.The results illustrated that 1) the problems and needs of patients were the physical problems of heart palpitations, dyspnea, dizziness, fatigue, and bleeding tendency; 2) lack of self-care knowledge; and 3) fear of communicating with personnel and society. Therefore, patients needed care for these problems. 2) The clinical nursing practice guidelines that were developed included steps and definitions of care, nursing diagnosis, objectives, implementations, and evaluation. The quality assessments of the CNPG by AGREE II were 92.86% overall and 90.00–97.78% in each item. 3) After CNPG testing, the knowledge and self-care behaviors of patients were significantly improved at .05. การวิจัยนี้มีวัตถุประสงค์ 1) เพื่อศึกษาปัญหาและความต้องการของผู้ป่วยโรคความดันโลหิตสูงที่มีหัวใจห้องบนพลิ้วไหว 2) เพื่อพัฒนาแนวปฏิบัติการพยาบาลทางคลินิกในการดูแลผู้ป่วยโรคความดันโลหิตสูงที่มีหัวใจห้องบนพลิ้วไหวในชุมชน และ 3) เพื่อศึกษาประสิทธิผลของแนวปฏิบัติการพยาบาลทางคลินิกในการดูแลผู้ป่วยโรคความดันโลหิตสูงที่มีหัวใจห้องบนพลิ้วไหวในชุมชน รูปแบบการวิจัยและพัฒนามีขั้นตอนการพัฒนาแนวปฏิบัติการพยาบาลทางคลินิกตามแนวคิดของซูคัพ กลุ่มตัวอย่างแบ่งตาม 4 ขั้นตอน ได้แก่ 1) การกระตุ้นเพื่อค้นหาปัญหาและแนวปฏิบัติทางคลินิก กลุ่มตัวอย่างมี 2 กลุ่ม ได้แก่ ผู้ป่วยและพยาบาลวิชาชีพ จำนวนกลุ่มละ 12 คน 2) การสนับสนุนและค้นหาหลักฐานเชิงประจักษ์ กลุ่มตัวอย่างคือตำราและงานวิจัย จำนวน 11 เรื่อง พยาบาลวิชาชีพ จำนวน 12 คน และผู้ทรงคุณวุฒิ 5 คน 3) การนำแนวปฏิบัติการพยาบาลทางคลินิกไปทดลองใช้ กลุ่มตัวอย่าง ได้แก่ ผู้ป่วยและพยาบาลวิชาชีพ จำนวนกลุ่มละ 15 คน และ 4) การนำแนวปฏิบัติการพยาบาลทางคลินิกไปใช้ในการปฏิบัติงาน กลุ่มตัวอย่างถูกเลือกแบบเจาะจง เครื่องมือการวิจัยประกอบด้วย 1) แนวคำถามการสนทนากลุ่มเพื่อศึกษาปัญหาและความต้องการของผู้ป่วย 2) แนวทางการระดมสมองเพื่อสร้างแนวปฏิบัติการพยาบาลทางคลินิก 3) แบบประเมินคุณภาพของแนวปฏิบัติการพยาบาลทางคลินิกอะกรีทู 4) แบบสอบถามความรู้และพฤติกรรมการดูแลตนเองของผู้ป่วย และ 5) แบบสอบถามความพึงพอใจในการใช้แนวปฏิบัติการพยาบาลทางคลินิกสำหรับพยาบาลวิชาชีพ วิเคราะห์ข้อมูลโดยด้วยสถิติเชิงพรรณนา การวิเคราะห์เชิงเนื้อหา และสถิติทดสอบวิลล์คอกซัน ไซน์ แรงค์ผลการวิจัยพบว่า 1) ปัญหาและความต้องการของผู้ป่วย ได้แก่ ใจสั่น หายใจหอบเหนื่อย เวียนศีรษะ อ่อนเพลีย มีภาวะเลือดออกง่าย ความเข้าใจเกี่ยวกับการดูแลตนเองไม่ถูกต้อง ความกลัวในการสื่อสารกับเจ้าหน้าที่และการเข้าสังคม ผู้ป่วยจึงต้องการการดูแลตามปัญหาเหล่านี้ 2) แนวปฏิบัติการพยาบาลทางคลินิกประกอบด้วย ขั้นตอนและคำจำกัดความในการดูแล ข้อวินิจฉัยทางการพยาบาล วัตถุประสงค์ กิจกรรมและการประเมินผลพยาบาล ซึ่งการประเมินคุณภาพของแนวปฏิบัติการพยาบาลทางคลินิกตามวิธีของอะกรี ทู ในภาพรวมได้ร้อยละ 92.86 และรายหมวดได้ร้อยละ 90.00-97.78 และ 3) หลังทดลองใช้แนวปฏิบัติการพยาบาลทางคลินิก พบว่าผู้ป่วยมีความรู้และพฤติกรรมการดูแลตนเองดีขึ้นอย่างมีนัยสำคัญทางสถิติที่ .05 แต่ผลลัพธ์ทางคลินิกไม่มีความแตกต่างอย่างมีนัยสำคัญทางสถิติที่ .05 ส่วนพยาบาลวิชาชีพที่ใช้แนวปฏิบัติการพยาบาลทางคลินิกมีความพึงพอใจอยู่ในระดับมากที่สุด
