Factors Influencing the Implementation of Hospital Accreditation and Continuous Quality Improvement of Community Hospitals in the 8th Health Region in Office of the Permanent Secretary Ministry of Public Health of Thailand

Abstract

This research’s objectives were to identify (1) factors in the operations of the Hospital Accreditation (HA) for quality assurance and development program and (2) factors influencing the operations of the HA program, at small-size community hospitals in Health Region 8 under the Office the Permanent Secretary of the Ministry of Public Health.This is survey research. The population consisted of personnel working in nine small-size community hospitals that had not received HA certification in Health Region 8. Out of all 816 hospital personnel, 389 officials were selected using stratified random sampling. Data were collected using a questionnaire concerning personal factors, structural factors, process factors, and the results of HA certification requests. The questionnaire’s validity and reliability values were 0.91 and 0.87, respectively. The statistics used were descriptive statistics and multiple regression analysis.The results showed that: (1) concerning personal factors, of all participants, the majority or 78.15% were female, 82.52% had experience in HA or quality development work not exceeding 5 years, and 64.78% had received training in quality development, and (2) structural factors (leadership and communications) and process factors (access to service recipients’ information) influenced HA or continuous quality assurance operations.

การวิจัยนี้มีวัตถุประสงค์เพื่อศึกษา (1) ปัจจัยในการดำเนินงานการรับรองและพัฒนาคุณภาพโรงพยาบาลชุมชน และ (2) ปัจจัยที่มีอิทธิพลต่อการดำเนินงานรับรองคุณภาพและ การพัฒนาคุณภาพอย่างต่อเนื่องของโรงพยาบาลชุมชนขนาดเล็ก เขตสุขภาพที่ 8 สำนักงานปลัดกระทรวงสาธารณสุขการวิจัยนี้เป็นการวิจัยเชิงสำรวจ ประชากรคือ บุคลากรที่ปฏิบัติงานในโรงพยาบาลชุมชนขนาดเล็ก 9 แห่งที่ไม่ผ่านการรับรองคุณภาพ ในเขตสุขภาพที่ 8 จำนวน 816 คน กลุ่มตัวอย่าง โดยการสุ่มแบบแบ่งชั้น จากการคำนวณได้ 389 คน เครื่องมือคือแบบสอบถาม ประกอบด้วยปัจจัยส่วนบุคคล ปัจจัยด้านโครงสร้าง ปัจจัยด้านกระบวนการ และผลลัพธ์ การรับรองและพัฒนาคุณภาพอย่างต่อเนื่อง มีค่าความตรงที่ 0.91 และความเที่ยงที่ 0.87 สถิติที่ใช้คือ สถิติเชิงพรรณนา และการวิเคราะห์ถดถอยเชิงเส้นพหุคูณผลการศึกษาพบว่า (1) ปัจจัยส่วนบุคคลส่วนมากเป็นเพศหญิงร้อยละ 78.15 มีประสบการณ์ในการทำงานคุณภาพไม่เกิน 5 ปีร้อยละ 82.52 และเข้ารับการอบรมในการพัฒนาคุณภาพ ร้อยละ 64.78 และ (2) ปัจจัยด้านโครงสร้างได้แก่ ด้านผู้นำและการสื่อสารมีอิทธิพลต่อการดำเนินงานรับรองคุณภาพและการพัฒนาคุณภาพอย่างต่อเนื่อง และปัจจัยด้านกระบวนการ ด้านการเข้าถึงข้อมูลผู้รับบริการ มีอิทธิพลต่อการดำเนินงานรับรองคุณภาพและการพัฒนาคุณภาพอย่างต่อเนื่อง
