| dc.description.abstract | การวิจัยเชิงพรรณานี้มีวัตถุประสงค์เพื่อ (1) วิเคราะห์ต้นทุนกิจกรรมการบริการตรวจวินิจฉัยภาวะสมองเสื่อมต่อราย (2) วิเคราะห์ต้นทุนต่อกิจกรรม และ (3) เปรียบเทียบต้นทุนการบริการกับรายได้ที่เรียกเก็บตามสิทธิการรักษาของผู้ป่วยที่ได้รับการวินิจฉัยภาวะสมองเสื่อม งานผู้ป่วยนอก โรงพยาบาลประสาทเชียงใหม่ประชากร คือ ข้อมูลผู้รับบริการการตรวจวินิจฉัยภาวะสมองเสื่อมต่อปี จำนวน 170 ราย กลุ่มตัวอย่าง คือ ข้อมูลผู้ป่วยจำนวน 45 ราย จากการสุ่มตามช่วงเวลาระหว่างวันที่ 1 ตุลาคม พ.ศ. 2561 ถึงวันที่ 31 ธันวาคม พ.ศ. 2561 เครื่องมือที่ใช้ในการวิจัย คือ พจนานุกรมกิจกรรมบริการการตรวจวินิจฉัยภาวะสมองเสื่อมแบบบันทึกต้นทุนลงทุนและต้นทุนดำเนินการ ซึ่งใช้เก็บข้อมูลปฐมภูมิด้านเวลาและปริมาณงานกิจกรรม และข้อมูลทุติยภูมิที่เป็นต้นทุนจากแผนกการเงิน งานพัสดุ งานแผนและนโยบาย และงานธุรการของโรงพยาบาลประสาทเชียงใหม่ วิเคราะห์ข้อมูล โดยการหาค่าเฉลี่ยและร้อยละ ผลการศึกษาพบว่า (1) ต้นทุนรวมกิจกรรมบริการการตรวจวินิจฉัยภาวะสมองเสื่อมต่อราย ณ โรงพยาบาลประสาทเชียงใหม่ มีค่าเท่ากับ 4,646.68 บาท คิดเป็นสัดส่วนต้นทุนค่าแรง : ต้นทุนค่าวัสดุ : ต้นทุนค่าลงทุน เท่ากับ 95.41 : 4.23 : 0.36 (2) ต้นทุนต่อกิจกรรม ได้แก่ กิจกรรมการซักประวัติอาการ การเตรียมผู้ป่วยก่อนการคัดกรองเบื้องต้นมีต้นทุน 641.96 บาท กิจกรรมการใช้เเบบคัดกรองภาวะสมองเสื่อมมีต้นทุน 1,139.70 บาท กิจกรรมการตรวจวินิจฉัยโดยอายุรแพทย์ระบบประสาทมีต้นทุน 2,004.08 บาท และกิจกรรมการแนะนำการปฏิบัติตัว และการมาตรวจตามนัดมีต้นทุน 594.28 บาท คิดเป็นสัดส่วนต้นทุนกิจกรรมก่อนพบแพทย์ : กิจกรรมขณะพบแพทย์ : กิจกรรมหลังพบแพทย์ เท่ากับ 3 : 3.7 : 1 และ (3) ค่าเฉลี่ยการตรวจวินิจฉัยภาวะสมองเสื่อมต่อรายรวมทุกสิทธิการรักษา เท่ากับ 4,040.01 บาท ซึ่งปัจจุบันการเบิกจ่ายหรืองบประมาณที่ถูกจัดสรรสำหรับการตรวจวินิจฉัยภาวะสมองเสื่อมแบบผู้ป่วยนอก ในผู้ป่วยสิทธิหลักประกันสุขภาพถ้วนหน้า โรงพยาบาลสามารถเบิกจ่ายต่อครั้งได้ตามจริงแต่ไม่เกิน 650 บาท สำหรับผู้ป่วยสิทธิประกันสังคมและผู้ป่วยที่เบิกค่ารักษาพยาบาลได้โรงพยาบาลสามารถเรียกเก็บได้ตามค่าใช้จ่ายจริง | th_TH |

| dc.description.abstractalternative | This is a descriptive study, whose objectives were: (1) to determine the cost of dementia diagnostic services per patient; (2) to determine the cost per dementia diagnostic activity; and (3) to compare the cost of dementia diagnostic services with the revenue collected according to the health insurance eligibility of the patients who had been diagnosed with dementia in the outpatient unit at Chiang Mai Neurological Hospital. The study involved a random sample of 45 patients’ data, randomly selected out of 170 annually diagnosed dementia patients, from 1 October through 31 December 2018. Data were collected using the dementia diagnostic services activity dictionary and the investment and operational cost recording forms for primary data on time spent and numbers of activities, and secondary data on investment costs from the hospital’s finance, supplies, policy and administrative sections. The data were then analyzed to determine means and percentages. Results showed that: (1) The overall average cost of dementia diagnostic services per patient was 4,646.68 baht; and the ratio of labor cost to material cost to capital cost was 95.41:4.23:0.36; (2) The cost per activity was 641.96 baht for history- taking and patient preparation before screening, 1,139.70 baht for dementia screening, 2,004.08 baht for diagnosis by a neurologist, and 594.28 baht for self-care and appointment adherence advice. The ratio of the preliminary screening cost to diagnostic stage cost to post-diagnosis cost was 3:3.7:1; and (3) The average cost of dementia diagnostic services was 4,040.01 baht for one patient under any type of government health insurance schemes. However, under the Universal Coverage Scheme, the hospital could get reimbursed for the actual cost but not exceeding 650 baht, while the full actual cost could be reimbursed for those under the Social Security Scheme and the Civil Servant Benefit Scheme | en_US |
