| dc.description.abstract | การเปรียบเทียบและประสานรายการยา เป็นวิธีการปฏิบัติที่สำคัญอย่างหนึ่งของระบบการจัดการด้านยา โดยเฉพาะในช่วงรอยต่อของการรักษา ทำให้ผู้ป่วยได้รับยาที่สมควรได้รับอย่างเหมาะสมและครบถ้วน ไม่เกิดการใช้ยาซ้ำซ้อนหรือการได้รับยาที่ไม่จําเป็น การจัดทําแนวปฏิบัติในการเปรียบเทียบและประสานรายการยา แผนกผู้ป่วยใน สําหรับบุคลากรทางการแพทย์ โรงพยาบาลบึงสามพัน จังหวัดเพชรบูรณ์ จะช่วยให้กระบวนการดำเนินงานเป็นไปตามมาตรฐาน ช่วยลดความคลาดเคลื่อนทางยาที่อาจจะเกิดขึ้น ส่งผลให้ผู้ป่วยได้รับความปลอดภัยจากการใช้ยา ขั้นตอนการจัดทำแนวปฏิบัติฯ ได้แก่ (1) การทบทวนเอกสารและงานวิจัยที่เกี่ยวข้อง (2) จัดทำร่างแนวปฏิบัติในการเปรียบเทียบและประสานรายการยา แผนกผู้ป่วยใน สำหรับบุคลากรทางการแพทย์ โรงพยาบาลบึงสามพัน (3) การตรวจสอบความตรงด้านเนื้อหาของร่างแนวปฏิบัติ โดยผู้ทรงคุณวุฒิ 3 ท่าน ได้ค่าดัชนีความสอดคล้อง เท่ากับ 0.98 และปรับแก้ไขตามคำแนะนำ (4) นำแนวปฏิบัติฯ ไปทดลองใช้กับบุคลากรทางการแพทย์ แผนกผู้ป่วยใน โรงพยาบาลบึงสามพัน จำนวน 33 คน เป็นระยะเวลา 1 สัปดาห์ และ (5) ประเมินความพึงพอใจต่อการใช้แนวปฏิบัติฯ ผลการดำเนินงาน ได้แนวปฏิบัติในการเปรียบเทียบและประสานรายการยา แผนกผู้ป่วยใน สำหรับบุคลากรทางการแพทย์ โรงพยาบาลบึงสามพัน เนื้อหาประกอบด้วย (1) บทนำ (2) ความรู้เกี่ยวกับมาตรฐานระบบยา (3) ความรู้เกี่ยวกับการเปรียบเทียบและประสานรายการยา (4) ความรู้เกี่ยวกับรอยต่อการดูแลรักษา (5) ความรู้เกี่ยวกับความคลาดเคลื่อนทางยาที่เกี่ยวข้องกับการเปรียบเทียบและประสานรายการยา (6) บทบาทและหน้าที่ของบุคลากรทางการแพทย์ในกระบวนการเปรียบเทียบและประสานรายการยา และ (7) แนวปฏิบัติของบุคลากรทางการแพทย์ในกระบวนการเปรียบเทียบและประสานรายการยา สําหรับผลการทดลองใช้ พบว่า บุคลากรทางการแพทย์มีความพึงพอใจต่อการใช้แนวปฏิบัติฯ โดยรวมในระดับมากที่สุด สำหรับความพึงพอใจรายด้าน พบคะแนนเฉลี่ยสูงสุด 3 ลำดับแรก ได้แก่ สามารถนำแนวปฏิบัติไปใช้เป็นแนวทางในการปฏิบัติงานได้ รองลงมา คือ ความชัดเจนของข้อมูลเนื้อหา และความเหมาะสมของภาษาที่ใช้ | th_TH |

| dc.description.abstractalternative | Medication reconciliation has been indicated as one of the important practices or standards of the medical management system, especially during the transition of care process. This is to ensure that the patients receive the medication they deserve appropriately and completely, no repeated drug use or unnecessary medications. Having guidelines of inpatient department medication reconciliation for medical personnel at Buengsamphan Hospital in Phetchabun province would help the operation process to be in accordance with the standards, reduce potential medication errors, and ensure patient’s medication safety. Procedures for creating the guidelines included: (1) reviewing relevant documents and research reports; (2) drafting guidelines of inpatient department medication reconciliation for medical personnel at the hospital; (3) getting the content validity of the draft guidelines checked by three experts, resulting in an overall item-objective congruence index of 0.98; and revising the guidelines according to recommendations; (4) pre pre-testing the guidelines for one week among 33 medical personnel working in inpatient department of the hospital; and (5) then assessing their satisfaction with the guidelines. As a result of the study, the guidelines of inpatient department medication reconciliation for medical personnel at Buengsamphan Hospital was finalized, consisting of; (1) Introduction, (2) Knowledge about medical management (3) Knowledge of medication reconciliation, (4) Knowledge about transition of care, (5) Knowledge about medication errors related to medication reconciliation, (6) Roles and duties of medical personnel in medication reconciliation processes, and (7) Guide lines for medical personnel when using medication reconciliation processes at Buengsamphan Hospital. As for the guidelines trial results, it was found that the medical personnel had an average score for overall satisfaction at the highest level.
