| dc.description.abstract | การวิจัยนี้เป็นกึ่งทดลอง 1 กลุ่มวัดก่อนและหลังการทดลอง มีวัตถุประสงค์ 1) เพื่อศึกษาชนิดและลักษณะของความคลาดเคลื่อนทางยาของงานบริการจ่ายยาแผนกผู้ป่วยนอกและใน 2) หาปัจจัยที่เป็นสาเหตุของการเกิดความคลาดเคลื่อนทางยาและ 3) เปรียบเทียบชนิด ลักษณะ ความคลาดเคลื่อนทางยาก่อนและหลังให้โปรแกรมการบริหารความเสี่ยงโรงพยาบาลเฉพาะทางด้านมะเร็งภาคใต้ ประชากรที่ศึกษาคือจำนวนใบสั่งยาทั้งปี จากงานบริการจ่ายยาแผนกผู้ป่วยนอก12,228 ใบ และผู้ป่วยใน 20,858 ใบ ในช่วงเวลา 1 ก.ค. -31 ธ.ค. 2558 กลุ่มตัวอย่างสุ่มแบบง่ายจำนวนใบสั่งยา จากแผนกผู้ป่วยนอก 3,040 ใบ และผู้ป่วยใน 5,026 ใบ เครื่องมือที่ใช้คือ แบบฟอร์มและโปรแกรมบันทึกความคลาดเคลื่อนทางยา และโปรแกรมบันทึกความเสี่ยงความคลาดเคลื่อนทางยาคุณภาพของเครื่องมือคือการควบคุมใบบันทึกการใช้ยาผู้ป่วยใน ทุกใบในระบบการจ่ายยามีกระบวนการดำเนินการเหมือนกันทุก สถิติที่ใช้คืออัตราส่วน และร้อยละ ผลการศึกษาพบว่า 1) ในงานบริการจ่ายยาแผนกผู้ป่วยนอกและใน ทั้งก่อนและหลังให้โปรแกรมการบริหารความเสี่ยงเกิดจากกระบวนการก่อนจ่ายยามากที่สุด 2) ปัจจัยที่มากที่สุดที่ทำให้เกิดความคลาดเคลื่อนทางยาในทุกๆขั้นตอนคือไม่มีการตรวจสอบซ้ำ และ3) หลังให้โปรแกรมบริหารความเสี่ยงของงานบริการจ่ายยาแผนกผู้ป่วยนอกแล้วพบว่า ความคลาดเคลื่อนทางยาโดยรวมทุกขั้นตอนลดลงร้อยละ 30.40 การสั่งใช้ยา กระบวนการก่อนจ่ายยา การจ่ายยาลดลงร้อยละ21.98, 24.34 และ 56.68 ตามลำดับ ส่วนงานบริการจ่ายยาแผนกผู้ป่วยใน พบว่า ความคลาดเคลื่อนทางยาโดยรวมทุกขั้นตอนลดลงร้อยละ 52.86 จากการสั่งใช้ยาคัดลอกคำสั่งใช้ยากระบวนการก่อนจ่ายยาการจ่ายยาลดลงร้อยละ 70.10, 49.85, 46.22 และ 62.29 ตามลำดับ | th_TH |

| dc.description.abstractalternative | This one group pre-post, quasi-experimental study aims to 1) determine types and characteristics of errors for the Out-patient and the In-patient medication dispensing services; 2) analyze factors accounting for medication dispensing errors; and 3) compare characteristics of medication errors in drug dispensing services of the Out-patient and the In-patient Departments at pre- and post- implementations of the risk management program at the Southern Cancer Hospital. The population was the prescriptions issued between 1 July and 31 December 2015--12,228 for outpatients, and 20,858 for inpatients. A sample of 3,040 prescriptions was drawn by a random sampling technique. The research instruments were a medication errors record form, an incident occurrence report program, and a risk management on medication errors program.
