The Development of a Triage Model for Emergency Patients Using a Smartphone Application

Abstract

The objectives of this research were: (1) to explore the emergency triage situation, (2) to develop a smartphone application model for emergency patient triage, and (3) to study the effect of using the developed emergency patient triage model.This research and development project was divided into three phases. For Phase 1, studying the emergency triage situation, eight key informants were selected by purposive sampling, consisting of 1 doctor, 1 supervisory nurse, and six staff nurses. For Phase 2, developing an emergency triage model using a smartphone application, the key informants were the same as in the first phase. For Phase 3, applying the model, the samples comprised 184 emergency patients which were gathered by systematic random sampling and calculated using G power Data were analyzed using descriptive statistics and Mann-Whitney U test. The research results were as follows. 1) Emergency triage situation- (1) Structure: The emergency triage was not comprehensive for all emergency conditions. The information system was not interconnected; and nurses lacked knowledge, skills, and expertise in emergency triage. (2) Process: The processes of triage were inconsistent and were not unified among nurses. (3) Outcome: The emergency triage was incorrect and fell below standard criteria. 2) The developed emergency triage model was designed as the following- (1) Structure: The model was an application based on a smartphone platform and using the emergency triage criteria of the National Institute of Emergency Medicine. The interface was user-friendly, and utilized a dropdown menu under a set of problem-solving commands. (2) Process: The method of emergency triage became easier through predefined lists within the application. (3) Outcome: The application evaluated the time and accuracy of emergency triage. 3) Average time for emergency triage using the created model was 50.85 seconds. The accuracy of emergency triage with this model was statistically significantly higher than the traditional one at the level of .001.

การศึกษาวิจัยครั้งนี้ มีวัตถุประสงค์เพื่อ 1) ศึกษาสถานการณ์การคัดแยกผู้ป่วยฉุกเฉิน 2) พัฒนารูปแบบการคัดแยกผู้ป่วยฉุกเฉินด้วยแอปพลิเคชันผ่านสมาร์ทโฟน และ 3) ศึกษาผลของการใช้รูปแบบการคัดแยกผู้ป่วยฉุกเฉินฯ ที่พัฒนาขึ้น การวิจัยและพัฒนานี้มีกลุ่มตัวอย่างแบ่งออกเป็น 3 ระยะการวิจัย คือ ระยะที่ 1 การศึกษาสถานการณ์การคัดแยกผู้ป่วยฉุกเฉิน ผู้ให้ข้อมูลหลักได้จากการคัดเลือกแบบเฉพาะเจาะจง ประกอบด้วยแพทย์ 1 คน พยาบาลหัวหน้างาน 1 คน และพยาบาล 6 คน รวม 8 คน ระยะที่ 2 การพัฒนารูปแบบการคัดแยกผู้ป่วยฉุกเฉินด้วยแอปพลิเคชันผ่านสมาร์ทโฟน เป็นกลุ่มตัวอย่างเดียวกันกับระยะที่ 1 และระยะที่ 3 การทดลองใช้รูปแบบการคัดแยกผู้ป่วยฉุกเฉินที่พัฒนาขึ้น เป็นเวชระเบียนผู้ป่วยฉุกเฉินได้จาการสุ่มเลือกอย่างเป็นระบบ คำนวณกลุ่มตัวอย่างโดยใช้สูตรการคำนวณขนาดกลุ่มตัวอย่างด้วยโปรแกรม G power จำนวน 184 คน วิเคราะห์ข้อมูลโดยสถิติพรรณนา และสถิติทดสอบแมน วิทนีย์ ยู ผลการศึกษา มีดังนี้ 1) สถานการณ์การคัดแยกผู้ป่วยฉุกเฉิน ได้แก่ (1) ด้านโครงสร้าง การคัดแยกผู้ป่วยฉุกเฉินไม่ครอบคลุมภาวะฉุกเฉิน ระบบสารสนเทศยังไม่เชื่อมโยงกัน พยาบาลขาดความรู้ทักษะความชำนาญในการคัดแยกผู้ป่วยฉุกเฉิน (2) ด้านกระบวนการ กระบวนการคัดแยกพยาบาลปฏิบัติแตกต่างไม่เป็นทิศทางเดียวกัน และ (3) ด้านผลลัพธ์ การคัดแยกผู้ป่วยฉุกเฉินไม่ถูกต้องและต่ำกว่าเกณฑ์มาตรฐาน 2) รูปแบบการคัดแยกผู้ป่วยฉุกเฉินที่พัฒนาขึ้น มีลักษณะดังนี้ (1) ด้านโครงสร้าง เป็นแอปพลิเคชันบนสมาร์โฟนคัดแยกผู้ป่วยฉุกเฉินตามเกณฑ์การคัดแยกผู้ป่วยฉุกเฉินของสถาบันการแพทย์ฉุกเฉินแห่งชาติ ขั้นตอนการใช้งานง่าย และใช้รายการเลือกแบบดึงลงภายใต้ชุดคำสั่งของการแก้ปัญหา (2) ด้านกระบวนการ วิธีการคัดแยกผู้ป่วยฉุกเฉินทำได้ง่ายขึ้นตามรายการที่กำหนดในแอปพลิเคชัน (3) ด้านผลลัพธ์ คือ ประเมินระยะเวลาและความถูกต้องของการคัดแยกผู้ป่วยฉุกเฉิน และ 3) ผลการใช้รูปแบบการคัดแยกผู้ป่วยฉุกเฉินด้วยรูปแบบการคัดแยกผู้ป่วยฉุกเฉินที่พัฒนาขึ้นฯใช้ระยะเวลาสั้นกว่า 50.85 วินาทีเมื่อเปรียบเทียบกับการคัดแยกผู้ป่วยฉุกเฉินรูปแบบเดิม 2.36 เท่า (120 วินาที) และการคัดแยกผู้ป่วยฉุกเฉินด้วยรูปแบบการคัดแยกผู้ป่วยฉุกเฉินนี้มีความถูกต้องมากกว่ารูปแบบเดิมอย่างมีนัยสำคัญทางสถิติที่ระดับ .001
