| dc.description.abstract | การศึกษาครั้งนี้มีวัตถุประสงค์เพื่อศึกษา (1) ประเภทความเสี่ยงของโรงพยาบาล (2) ขั้นตอน การจัดการความเสี่ยงของโรงพยาบาล และ (3) ออกแบบฟอร์มที่เกี่ยวข้องกับการจัดการความเสี่ยงของ โรงพยาบาลนอร์ทอีสเทอร์น-วัฒนา การศึกษาครั้งนี้เป็นการศึกษาเชิงพรรณนาโดยการเก็บรวบรวมข้อมูลต่างๆ ข้อมูลปฐมภูมิจากการสัมภาษณ์จากผู้ปฏิบัติงานจากประสบการณ์จริงในการทำงานในโรงพยาบาลนอร์ท อีสเทอร์น-วัฒนาและข้อมูลทุติยภูมิจากข้อกำหนดมาตรฐานการจัดการระบบคุณภาพรวมทั้งเอกสารต่าง ๆ ของโรงพยาบาลที่เกี่ยวข้องกับการจัดการความเสี่ยง ผลการศึกษาพบว่า (1) ประเภทของความเสี่ยงของโรงพยาบาลมี 7 ประเภท คือ 1) ความเสี่ยง ด้านสิทธิผู้ป่วยจริยธรรมองค์กร 2) ความเสี่ยงด้านอาชีวอนามัยและความปลอดภัยในการทำงาน 3) ความเสี่ยงด้านการติดเชื้อในโรงพยาบาล 4) ความเสี่ยงด้านทรัพย์สินและอุปกรณ์ 5) ความเสี่ยงด้านความปลอดภัยของผู้ป่วย 6) ความเสี่ยงด้านการใช้ยา 7)ความเสี่ยงเกี่ยวกับการสื่อสาร(2) ขั้นตอนการจัดการความเสี่ยงของ โรงพยาบาลมี 7 ขั้นตอนดังนี้ 1) ผู้บริหารระดับสูง กำหนดนโยบายการจัดการความเสี่ยงของโรงพยาบาล 2) กำหนดวัตถุประสงค์และเป้าหมาย ในการจัดการความเสี่ยงของโรงพยาบาล 3) กำหนดตัวชี้วัด ในการจัดการความเสี่ยงของโรงพยาบาล 4) กำหนดโครงสร้างการบริหารความเสี่ยงของโรงพยาบาล 5) การจัดทำคู่มือปฏิบัติ 6) การปฏิบัติตามคู่มือ 7) การทบทวนคู่มือและประเมินผลการจัดการความเสี่ยง (3) การออกแบบฟอร์มที่ใช้ในการจัดการความเสี่ยงของโรงพยาบาลจำนวน 4 แบบฟอร์ม คือ 1) แบบฟอร์ม การรายงานอุบัติการณ์ 2) แบบฟอร์มสรุปมาตรการแนวทางและแผนการปฏิบัติ 3) แบบบันทึกการทำการทบทวนเหตุการณ์ขณะผู้ป่วยยังรักษาพยาบาล 4) แบบฟอร์มการทบทวน อุบัติการณ์โดยใช้การวิเคราะห์รากเหง้าของปัญหาโดยใช้ลำดับเวลาของเหตุการฟ้ซ็งการใช้แบบฟอร์ม เหล่านี้เบื้องด้นช่วยในการนำระบบจัดการความเสี่ยงสู่ภาคปฏิบัติได้อย่างมีประสิทธิภาพยี่งขึ้นเนื่องจากช่วยอำนวยความสะดวกแก่ผู้ปฏิบัติ | th_TH |

| dc.description.abstractalternative | The objectives of the study were (1) to study types of risks in the hospital; (2) to study risk management procedures in the hospital; and (3) to design the forms for the risk management system of North Eastern-Wattana Hospital. The study was the descriptive study. The primary data was collected by interviewing the staff working at North Eastern-Wattana Hospital. The secondary data was collected from the standard requirements of quality management system including other hospital’s documents concerning the risk management. The results showed that (1) there were seven types of risks in hospital: 1) the risks of patients’ rights and organizational ethic, 2) the risks of occupational health and safety, 3) the risks of infection in a hospital, 4) the risks of assets and equipment, 5) the risks of patients’ safety, 6) the risks of medical usage, and 7) the risks of communication; (2) the seven procedures of hospital risk management were 1) the policy of hospital risk management was set up by the management team; 2) the objectives and goals of hospital risk management were set up; 3) the key performance indicators for hospital risk management were set up; 4) the structure of hospital risk management was established; 5) the hospital risk management manual was developed; 6) the implementation of the hospital risk management manual was achieved; and 7) the manual was reviewed and the results of risk management were evaluated; and (3) the four forms of hospital risk management was designed: 1) the incident report form, 2) the incident reviews and risk prevention action plan form, 3) the record of present incident form, and 4) the time series of basic problem form.
