| dc.description.abstract | การวิจัยต้นทุนต่อหน่วยบริการของสถานพยาบาลในจังหวัดน่านนี้ มีวัตถุประสงค์เพื่อศึกษาต้นทุนต่อหน่วยบริการของสถานพยาบาลในจังหวัดน่านประจำปีงบประมาณ 2546 โดยเป็นวิธีการศึกษาต้นทุนต่อหน่วยบริการแบบวิธีลัด จากข้อมูลรายงานการเงินการคลังและรายงานกิจกรรมสำคัญตามแบบรายงาน 0110 รง. 5 ของโรงพยาบาลทุกแห่งในจังหวัดน่าน จำนวน 13 แห่ง และสถานีอนามัยจำนวน 147 แห่ง ผลการศึกษาพบว่ารายจ่ายทุกหมวดรายการยกเว้นงบลงทุน ของโรงพยาบาลทุกแห่งในจังหวัดน่าน มีต้นทุนค่าใช้จ่ายเฉลี่ยสูงกว่าโรงพยาบาลในระดับเดียวกันของเขตตรวจราชการกระทรวงสาธารณสุข เขต 9 และระดับประเทศ โดยโรงพยาบาลในจังหวัดน่านมีค่าใช้จ่าย จำนวน 396.80 ล้านบาท ซึ่งสูงกว่าค่าใช้จ่ายเฉลี่ยในระดับประเทศ ร้อยละ 9.85 (ค่าใช้จ่ายเฉลี่ยระดับประเทศ 357.72 ล้านบาท) และสูงกว่าค่าใช้จ่ายเฉลี่ยในระดับเขต 9 ร้อยละ 0.56 (ค่าใช้จ่ายเฉลี่ยเขต 9 เท่ากับ 394.58 ล้านบาท) ต้นทุนต่อหน่วยบริการผู้ป่วยในพบว่าค่าเฉลี่ยต้นทุนต่อหน่วยบริการผู้ป่วยของโรงพยาบาลในจังหวัดน่านเท่ากับ 5,675.94 บาท ซึ่งสูงกว่าค่าเฉลี่ยในระดับประเทศร้อยละ 11.70 (ค่าเฉลี่ยรายจ่ายต้นทุนผู้ป่วยในต่อหน่วยบริการระดับประเทศ 5,011.83 บาท) และมีค่าเฉลี่ยรายจ่ายรวมต่อหน่วยบริการสูงกว่าค่าเฉลี่ยในระดับเขต 9 ร้อยละ 1.83 (ค่าเฉลี่ยรายจ่ายต้นทุนผู้ป่วยในต่อหน่วยบริการเขต 9 เท่ากับ 5,571.91 บาท) เมื่อพิจารณาต้นทุนต่อหน่วยบริการผู้ป่วยนอก ของสถานีอนามัยในเครือข่ายโรงพยาบาลทุกแห่งจังหวัดน่านกับค่าเฉลี่ยระดับประเทศและเขต 9 พบว่าต้นทุนต่อหน่วยบริการของสถานีอนามัยจำนวน 5 เครือข่าย มีต้นทุนสูงกว่าค่าเฉลี่ยระดับประเทศ (ค่าเฉลี่ยต้นทุนต่อหน่วยบริการของสถานีอนามัยในระดับประเทศเท่ากับ 109.58 บาท) และพบว่ามีสถานีอนามัย 3 แห่ง ที่มีต้นทุนต่อหน่วยบริการสูงกว่าค่าเฉลี่ยในระดับเขต 9 (ค่าเฉลี่ยต้นทุนต่อหน่วยบริการของสถานีอนามัยในระดับเขต 9 เท่ากับ 124.67 บาท) ค่าต้นทุนต่อหน่วยบริการของการศึกษาในครั้งนี้จะมีค่าสูงกว่าค่าที่ใช้ในการคิดงบหลักประกันสุขภาพถ้วนหน้าแม้ว่าข้อมูลที่นำมาใช้ในการคำนวณเหมือนกันคือรายงาน 0110 รง.5 ทั้งนี้ เหตุผลประการหนึ่งอาจเนื่องมาจากว่า การศึกษาในครั้งนี้ นำสัดส่วนต้นทุนผู้ป่วยในและผู้ปวยนอกสัดส่วนใหม่มาใช้ในการคำนวณหาต้นทุนผู้ป่วยนอกและผู้ป่วยใน ซึ่งมีค่าแตกต่างไปจากสัดส่วนเดิม จึงมีผลทำให้ค่าต้นทุนผู้ป่วยนอกและผู้ป่วยในที่ได้แตกต่างไปจากการศึกษางบหลักประกันสุขภาพถ้วนหน้า และการศึกษาในจังหวัดหนองบัวลำภู | th_TH |

| dc.description.abstractalternative | The objective of this study was to estimate unit cost of health care services in Nan province in the fiscal year 2003. The ethodology of this study was ca culated unit cost cy health care financial and activity report from 13 hospitals (general hospital and 12 community hospitals) and 147 health centers in fiscal year 2003. The results of this study indicated that average all of cost expense which excepts capital cost of the hospitals in Nan province was higher than average cost expense of the national and public health inspection 9 region. The cost expense of hospita's in Nan provrce is 396.80 million bant which higher than national average cost expense 9.95 percent (national average cost experse was 357.72 million baht) and higher than average cost expense of public health inspection 9 region 0.56 percent (average cost expense of public health inspection 9 region was 394.58 million baht). As for unit cost of in-patient department, average in-patient cepartment of the hospital in Nan province was 5,675.94 baht which higher than national average unit cost 11.70 percent (average national in-patient department unit cost was 5,011.83 baht). While compared with average unit cost of 9" region, the average unit cost of in-patient department in Nan province was higher than average unit cost of 9 region 1.83 percent (average unit cost of in-patient department in 9 region was 5,571.91 baht) Considering to the outpatient department, unit cost of health center in 5 Connecting Unit Provider (CUP) was higher than average national unit cost (average national health center unit cost was 109.58 bant). The average unit cost of 3 health centers was higher than average unit cost of 9 region (average health center unit cast of 9 region was 124.67 baht). STANDARDS.... In conclusion, the unit cost in this study is higher than co-efficient for budget estimation in universal coverage scheme. Although both model use the same source of data which is 0110 financial report of ministry of public health. As for the reason, both unit cost of inpatient and out-patient services in this study uses new co-efficient for unit cost calculation which different fram at first co-efficient. Therefore, both unit cost of inpatient and out-patient in this study is differ from the study of budget estimation in universal coverage scheme and the study of unit cost in universal coverage scheme-a case study in Knongbualumphu province | en_US |
